For those students needing a committee letter of recommendation for medical, dental, optometry, etc. schools, our school provides a "composite letter".
A true "committee letter" is one letter written by a group of individuals. We do not have this type.
In place of the "committee letter", a composite letter can be sent to the graduate school of choice. This letter is comprised of a cover-letter and three to five individually written letters. This "composite letter" is accepted by many medical institutions, in place of the "committee letter". I, as the chair, do not have to be one of the letter writers, yet it would usually be advisable.
In order to have a composite letter sent to a graduate school, you must print the following form:
